Heywood Rural Health is seeking a dedicated Physiotherapist to deliver high-quality, evidence-based care across our Residential Aged Care, Acute Services, and outreach programs. In this rewarding role, you will support consumers to maintain or improve their independence through person-centred, reablement-focused physiotherapy. You’ll collaborate closely within a multidisciplinary team, contribute to care planning, and ensure documentation and service delivery align with organisational and accreditation standards. Key Responsibilities:

  1. To provide evidenced based, clinically governed, person directed physiotherapy services to referred consumers of the Residential Aged Care and Acute Services,, using a wellness and reablement approach, centered on consumer needs, wants and choices;
  2. Define, monitor and adapt with consumers: Episodes Of Care (EOC), consumer directed goals, and care planning, with evidenced examples to measure outcomes of each intervention;
  3. To provide physiotherapy services both within the facility and also as an outreaching service, as outlined by the relevant service provision agreements, and in consultation with the Director of Clinical Services.
  4. Contribute as an active member of the multi-disciplinary health care team to achieve optimal outcomes for consumers;
  5. Maintain accurate and timely documentation in accordance with Heywood Rural Health requirements and accreditation standards on all occasions;
  6. Maintain and report to relevant committee as guided by Director of Clinical Services
  7. Contribute to the development and review of physiotherapy services, using reoccurring evaluation methods, and be actively involved in creating and monitoring physiotherapy key performance indicators (KPI’s);
  8. Actively identify and contribute to quality improvement activities and accreditation requirements within the Aged Care and Acute Services;
  9. Work collaboratively with Allied Health Assistant (AHA), to provide physiotherapy directed interventions with consumers;
  10. Flexibility in availability (of hours work may be required);
  11. Supervision of students, including work experience, undergraduate nursing students and undergraduate physiotherapy students (in consultation with the Director of Clinical Services).

Key Selection Criteria:

Essential:

  1. Bachelor of Applied Science (Physiotherapy) or equivalent
  2. Current AHPRA registration
  3. Experience in aged care and/or acute care physiotherapy
  4. Ability to work in a multidisciplinary team
  5. Excellent communication and documentation skills

Desirable

  1. Experience in outreach services
  2. Supervising students or AHAs
  3. Knowledge of quality improvement processes
