About the role
We are seeking a dedicated and capable Community Worker to join our growing team in Newcastle NSW. This role is focused on supporting clients through structured community based programs, needs assessments, progress monitoring, referral coordination, and liaison with families, service providers, community organisations, and relevant stakeholders.
The successful candidate will play an important role in helping clients access the services, resources, and support they need to improve their wellbeing, strengthen independence, and participate more actively in the community.
Key responsibilities
- Assess clients’ needs and identify barriers affecting their health, housing, training, employment, welfare, and community participation
- Interview clients and assess the nature and extent of their difficulties, support needs, goals, and available resources
- Plan, develop, and implement individual support programs and community participation strategies tailored to client needs
- Monitor client progress against goals and prepare accurate case notes, reports, and updates
- Refer clients to appropriate agencies, community services, health providers, training bodies, and other organisations that can provide additional support
- Assess community needs and available resources in relation to health, welfare, housing, employment, training, and related services
- Liaise with community groups, welfare agencies, government bodies, service providers, and private organisations to promote access to community resources and services
- Support clients and families by providing guidance, advocacy, education, and practical assistance in community and service settings
- Promote client independence, inclusion, and informed decision making through a person centred approach
- Contribute to the development of community awareness and service linkages that improve outcomes for people with disability and related support needs
Skills and experience
- Relevant qualification in Community Services, Disability, Mental Health, or a related field
- Experience working in community services, disability services, welfare support, or a related setting
- Strong skills in client assessment, support planning, documentation, and stakeholder liaison
- Ability to build professional relationships with clients, families, agencies, and community organisations
- Sound understanding of person centred practice, client dignity, confidentiality, and professional boundaries
- Excellent communication, organisational, and time management skills
- Current driver licence and access to a reliable vehicle
- Valid working rights in Australia
