Job Description

Community Worker

Daily tasks include:

1. Providing person-centred support to participants in their homes and within the community
2. Assisting with activities of daily living, including personal care, hygiene, dressing, and mobility
3. Supporting participants with meal preparation, nutrition, and attending appointments
4. Facilitating community access, including social, recreational, and skill-building activities
5. Monitoring participant wellbeing and reporting any changes in condition or behaviour
6. Maintaining accurate documentation, including progress notes and incident reports

Requirements:

Level of required qualification:

AQF Associate Degree, Advanced Diploma or Diploma (or equivalent experience can replace the qualification)

Number of years of experience:

At least 1 year of relevant experience in a similar role

Other main requirements:

1. Strong communication and interpersonal skills
2. Ability to work independently and adapt to varying environments
3. Understanding of person-centred practice and duty of care
4. Experience supporting individuals with disabilities, mental health conditions, or complex needs (preferred)
5. Current First Aid and CPR certification
6. Valid driver’s licence and ability to travel between client locations

Annual salary: $76,515 per annum + superannuation (Full-time, 38 hours per week)
