About the opportunity

We are currently seeking a committed Full-time (Monday to Friday) Clinical Care Coordinator to join our Clinical Leadership team at our Moran Sylvania Home. This role is one of two Clinical Care Coordinators at Moran Sylvania.

Reporting to the Care Manager you will lead a team of RN’s and achieve the best possible clinical outcomes for all residents by ensuring that high quality nursing services are provided through the application of clinical and nursing skills, practitioner knowledge, qualifications and work practice. You will participate in clinical governance activities, including planning and implementing clinical priorities.

Key Accountabilities:

  • Supervision of nursing care, services and clinical staff
  • Oversee specialised nursing care needs and high impact high prevalent risks
  • Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices
  • Provide clinical leadership, mentorship and assist ongoing clinical education
  • Establish and maintain positive relationships with residents, families, and health professionals
  • Support a culture of best practice, practice evaluation and achievement of clinical outcomes

Skills/Requirements:

  • Registered Nurse with at least 2 -3 years aged care clinical experience;
  • Strong leadership skills and knowledge of the Quality Standards for Aged Care
  • Excellent and demonstrated verbal, written and interpersonal communication skills.
  • Knowledge and ability to use computers and technology efficiently.
  • Current Police Check and appropriate Working Rights
  • Up to date vaccinations (Influenza, Covid-19)
  • Management qualifications (or working towards) – highly regarded
